Home Health RN Case Manager

Key Responsibilities ● Lead and collaborate to provide management and delivery of patient care plans ● Support patients in Jeanette, PA and surrounding areas ● Consistently meet critical deadlines for charting ● Self-manage time and schedule efficiently ● Deliver quality patient care Schedule and Shift Details ● Schedule (Including any on call/weekends): Mon-Fri, 8 hour days, 1 evening of on-call per week (5pm-10pm) and 1 weekend per month (Sat/Sun)

Qualifications

• Clinical Quality (1+ years)
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• Valid Registered Nurse (RN) in Pennsylvania (PA)
• Valid Regular Driver License (DL)
• ALL of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
• Nursing Experience (1+ years)
