Key Responsibilities ● Lead and support a team of dietitians and food service staff ● Guide the daily flow of on-site meal services ● Ensure food services meet PACE regulatory standards ● Conduct in-depth nutrition assessments ● Create individualized nutrition care plans ● Monitor key health signs like weight and hydration ● Provide nutrition education ● Collaborate with food service staff ● Review and approve meals ● Coordinate delivery of nutrition supplements and home meals ● Create wellness programs ● Run quality checks on food presentation and safety ● Lead changes to improve efficiency and satisfaction ● Provide training and learning opportunities ● Document nutrition stories in the EMR ● Communicate openly with participants and families ● Assist in food service or care support when needed ● Take on other duties as needed Schedule and Shift Details ● Full-time, Monday through Friday with a steady schedule. Travel ● May include occasional travel throughout the local market. Languages ● Bilingual skills in English and Spanish, Cantonese, Mandarin, or Vietnamese are a plus.

Qualifications

Required
  • Master's degree or higher
  • Valid Registered Dietitian Nutritionist (RDN) in California (CA)
  • Valid ServSafe Manager's Certification
  • Clinical Dietitian (3+ years)
  • Supervisory (1+ years)
  • Coordinating food services in a healthcare, senior care or community-based setting. (2+ years)
  • 1+ years of experience in ANY of the following:
    • Working with Frail Elderly
    • Working with Medically Complex Participants
    • Working with People from a Wide Variety of Backgrounds and Cultures.
