Overview

This role is ideal for a mission-driven physician who is passionate about empowering seniors to live independently. You will be a vital member of a collaborative team, providing holistic care tailored to the unique needs of PACE participants. Your work will directly impact the lives of older adults, helping them achieve their health goals. Key Responsibilities ● Serve as the primary care provider for a participant panel ● Deliver direct primary medical care to PACE participants ● Initiate, evaluate, review, and close orders for medications and treatments ● Provide care in various settings including PACE center, home, and skilled nursing facilities ● Coordinate care plans with IDT members ● Assist participants during hospital stays and nursing home transitions ● Educate participants and caregivers on medical care ● Initiate referrals to specialist providers ● Perform selected in-office procedures ● Maintain thorough and timely documentation in the EMR ● Gain exposure to Value-Based Care strategies ● Contribute to organizational growth Schedule and Shift Details ● On-call shifts and after-hour calls remotely on a rotating schedule. Languages ● Spanish proficiency is highly preferred.

Qualifications

Required
  • Doctorate degree
  • ANY of the following valid licenses/certifications:
    • Medical Doctor (MD) in California (CA)
    • Doctor of Osteopathic Medicine (DO) in California (CA)
  • ANY of the following valid licenses/certifications:
    • Board Certified
    • Board Eligible
  • Valid Controlled Dangerous Substances (CDS)
  • Valid Drug Enforcement Administration (DEA)
  • ANY of the following valid licenses/certifications:
    • American Red Cross Basic Life Support (ARC BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
    • American Heart Association Basic Life Support (AHA BLS)
  • Valid Regular Driver License (DL)
