Key Responsibilities ● Assist in orientation, training, and education of department members. ● Interview patients and families to gather relevant information. ● Act as a liaison between hospitalized patients, families, and community health agencies. ● Provide brief counseling services to patients and families. ● Maintain current information on community resources and financial benefits programs. ● Assist patients and families in post-discharge health care planning. ● Develop Policies & Procedures for specific Social Work needs. Schedule and Shift Details ● Monday to Friday, Outpatient setting.
Qualifications
- Master's degree or higher
- Utilization Review (1+ years)
- Social Work (1+ years)

- Licensed Independent Clinical Social Worker (LICSW) in New Mexico (NM)
