Join our client as a Home Care Physical Therapist, where you will provide high-quality patient care in the comfort of their homes. Key Responsibilities ● Makes visits to the homes of patients requiring home health physical therapy services ● Performs initial and comprehensive assessments on all patients ● Evaluates and assesses the patient’s level of functioning ● Treats clients to relieve pain and restore function ● Initiates diagnostic, preventative and rehabilitative procedures ● Ensures quality and safe delivery of services to patients ● Promotes patient independence through education ● Develops and updates a plan of care ● Maintains accurate and timely documentation of clinical records ● Communicates with all physicians involved in the plan of care ● Supervises home health aide and physical therapy assistant staff Schedule and Shift Details ● Monday-Friday 8am-4:30pm
Qualifications
- Bachelor's degree or higher
- Microsoft Office (1+ years)
- Valid Physical Therapist in Iowa (IA)
