Overview

Join our client as a Home Care Physical Therapist and make a meaningful impact on patients' lives. In this role, you will provide high-quality therapy services in patients' homes, ensuring their comfort and independence. You will work collaboratively with a dedicated team to develop personalized care plans and deliver exceptional patient experiences. Key Responsibilities ● Make visits to patients' homes for therapy services ● Perform initial and comprehensive assessments ● Evaluate and assess patients' functioning levels ● Treat clients to relieve pain and restore function ● Initiate diagnostic and rehabilitative procedures ● Ensure quality and safe delivery of services ● Promote patient independence through education ● Develop and update patient care plans ● Maintain accurate clinical documentation ● Communicate with physicians and healthcare practitioners ● Supervise home health aide and physical therapy assistant staff Schedule and Shift Details ● Monday-Friday 8am-4:30pm

Qualifications

Required
  • Bachelor's degree or higher
  • Home Health (1+ years)
  • Microsoft Office (1+ years)
  • Valid Physical Therapist in Iowa (IA)
