Key Responsibilities ● Collaborate with team to provide management and delivery of patient care plans ● Meet critical deadlines for charting ● Support patients in Williamsport, PA and surrounding areas Schedule and Shift Details ● Monday- Friday 8a-5p, 1 weekend every 4-6 weeks, 2 holidays per year Travel ● Ability to spend ~20% of your day driving to/from patient locations

Qualifications

Required
  • Bachelor's degree or higher
  • Home Health (1+ years)
  • 1+ years of experience in ANY of the following:
    • Electronic Medical Record (EMR)
    • Practice Partner EMR
    • EHR Implementation
  • Valid Regular Driver License (DL) in Pennsylvania (PA)
  • Valid Physical Therapist in Pennsylvania (PA)
  • Have you applied to or worked for Residential HH&H anytime in the past 6 months?
