Overview

Join our client as an LPN or LVN where you will play a vital role in ensuring the health and well-being of residents. You will provide leadership support and assist with resident care, making a positive impact in their lives. Key Responsibilities ● Monitor the health and well-being of residents ● Assist in training and monitoring medication administration ● Communicate with resident support services and families ● Maintain clinical quality assurance in accordance with federal, state, and local standards ● Conduct resident assessments and care plan updates ● Ensure proper follow through, assessment and documentation for residents ● Maintain frequent communications with residents’ families ● Order medications and equipment as needed ● Monitor medication administration and treatment profiles ● Attend regular meetings as directed Schedule and Shift Details ● Ability to work weekends, evenings and flexible hours.

Qualifications

• Associate degree or higher
• ANY of the following valid licenses/certifications:
  • Licensed Practical Nurse (LPN) in New Hampshire (NH)
  • Licensed Vocational Nurse (LVN) in New Hampshire (NH)
