Key Responsibilities ● Collaborate to provide management and delivery of patient care plans ● Support patients in Wexford and surrounding areas ● Consistently meet critical deadlines for charting ● Self-manage time and schedule efficiently ● Demonstrate experience with tablets, mobile phones, and EMR software Schedule and Shift Details ● Saturday & Sunday 8 AM - 8 PM Travel ● Local travel

Qualifications

Required
  • Associate degree or higher
  • Hospice (2+ years)
  • 1+ years of experience in ANY of the following:
    • Electronic Medical Record (EMR)
    • Other EMR
    • TruChart EHR
  • Valid Licensed Practical Nurse (LPN) in Pennsylvania (PA)
  • Valid Regular Driver License (DL)
