Wound Specialist LPN
Overview

Key Responsibilities ● Manage and deliver patient care plans collaboratively within a team ● Provide wound care services to patients in the coverage area ● Maintain accurate and timely charting and documentation ● Use EMR software and mobile devices for patient care management ● Drive to patient locations within the assigned coverage area Schedule and Shift Details ● Monday – Friday, 8:30 AM – 5:00 PM; no weekends, holidays, or on-call; typically sees 6-7 patients per day Travel ● Must live within 30 minutes of coverage area; approximately 20% driving to patient locations
Qualifications
- Associate degree or higher

1+ years of experience in ANY of the following:
- Home Health Care
- Home Health

1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- Practice Partner EMR
- Valid Licensed Practical Nurse (LPN) in Illinois (IL)
- Valid Certified Wound Ostomy and Continence Nurse (CWOCN)
- Valid Regular Driver License (DL) in Illinois (IL)
