Key Responsibilities ● Coordinate the plan of care focusing on quality, timely delivery of services, and medical necessity of patient admission ● Assess, plan, implement, monitor, and evaluate options and services to meet individual health care needs and family support systems ● Ensure well-orchestrated, coordinated care focused on quality, efficiency, and safety ● Maintain an open and approachable manner treating others fairly and respectfully, preserving self-confidence and dignity

Qualifications

Required
  • Bachelor's degree or higher
  • 1+ years of experience in ANY of the following:
    • Case Management
    • EHR Coordination
    • Care Management
  • Healthcare (1+ years)
  • Valid BSW in Kansas (KS)
