Director of Care Management
Overview
This leadership role offers the opportunity to guide care management teams in delivering patient-centered care across hospital and ambulatory settings. You will drive strategic initiatives to improve patient outcomes, optimize healthcare resources, and foster collaboration among clinical and community partners. This position is ideal for a leader passionate about advancing care management strategies and making a meaningful impact on healthcare delivery. Key Responsibilities ● Provide strategic leadership and operational oversight for Integrated Care Management programs across hospital and ambulatory settings ● Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers ● Build strong partnerships with clinical leaders, community organizations, and cross-functional stakeholders ● Drive process improvement initiatives to reduce readmissions, improve patient flow, optimize length of stay, and enhance care coordination outcomes ● Ensure compliance with federal and state regulatory requirements ● Coach, mentor, and develop team members fostering collaboration, accountability, and service excellence ● Analyze clinical, operational, and financial performance metrics to support strategic growth and quality improvement Schedule and Shift Details ● On-site work only
Qualifications
- Bachelor's degree or higher
- Care Management (1+ years)

1+ years of experience in ANY of the following:
- Sunrise Patient Financials Analytics
- Clinical Quality

ANY of the following valid licenses/certifications:
- Registered Nurse (RN) in Iowa (IA)
- Licensed Clinical Social Worker (LCSW) in Iowa (IA)
