Home Care Physical Therapist

Overview

This full-time Home Care Physical Therapist role offers an opportunity to deliver high-quality therapy services directly in patients' homes. You will use your clinical skills to assess, treat, and educate patients and their families, working collaboratively with healthcare teams to improve patient outcomes. The position includes a daytime schedule Monday through Friday and offers a sign-on bonus for external candidates. Key Responsibilities ● Visit patient homes to provide physical therapy services and conduct ongoing interdisciplinary assessments ● Perform initial and comprehensive patient assessments following agency policies ● Evaluate patient functioning using appropriate tests ● Provide treatment to relieve pain, restore function, and maintain performance per plan of care ● Initiate diagnostic, preventative, and rehabilitative procedures as appropriate ● Ensure quality and safe delivery of services according to care plan ● Use equipment and supplies safely and effectively ● Educate patients and families on treatment, disease management, medication use, equipment safety, emergency preparedness, and therapy phases ● Provide counseling and education to patients, caregivers, and families ● Develop and update individualized plans of care and discharge plans with measurable outcomes ● Include patients and caregivers in care planning and communicate plan revisions to all involved parties ● Maintain accurate and timely clinical documentation and time/travel records ● Obtain and document physician orders and communicate patient condition changes ● Coordinate with physicians and healthcare practitioners involved in patient care ● Identify and intervene to reduce patient risks for hospitalization or emergency visits ● Supervise home health aides and physical therapy assistants and document supervision Schedule and Shift Details ● Monday-Friday, 8am-4:30pm daytime hours Travel ● Travel to patient homes within the service area

Qualifications

• Bachelor's degree or higher
• Valid Physical Therapist in Iowa (IA)
