Patient Navigator / Nurse Liaison

Overview

We are searching for experienced RNs with excellent communication skills to join our team as a Nurse Navigator. In this role, you will coordinate and/or performs assessments and facilitates admission of patients referred to the facility. You’ll be working in collaboration with all the clinic team members to ensure that each patient receives the highest quality and safest care.
