Job Title: RN Case Manager Job Description: Client is seeking a dedicated RN to join the team as a Case Manager. The ideal candidate will be able to work as a hospice field nurse to provide both routine and emergency assessments as well as evaluative services to meet the needs of the patient and their family. Required skills & experience 1. Registered Nursing License in state of employment. 2. Minimum of one year nursing practice or equivalent experience. 3. Hospice, Oncology, Home Health and/or Long-Term Care experience What you need to know 1. Day Shift position (8:00 AM-5:00 PM) 2. 5-6 cases on average per day 3. Willing to participate in on call 3-5 days a month, including one weekend day/month. 4. Client prefers 1 + years of nursing practice experience but will train a new grad if they are the right fit. 5. Candidates will travel once a week from Gaylord, MI to the Atlanta, MI office (38 miles). 6. Targeted state date - ASAP 7. Position reports to Director, Clinical Services
