Responsibilities: You will independently perform home visits to conduct functional, psychosocial and environmental safety assessments, determine the eligibility of enrollees, and establish an individual plan of care that enables them to remain safely at home despite long-term care needs.Become a part of an innovative team. Close-knit, collaborative team; providing excellent quality care management. Work from your home + field work. Top "must-have" skills for this job – 1. Active NYS license - RN 2. Min 1 year of experience in acute med-surg care 3. Working knowledge of Medicare, Medicaid, and NYS DOH standards and regulations 4. BSN Preferred 5. Cantonese required 6.Position available in Brooklyn & Staten Island
