Job Description: As the Hospice Care Planner, you will be working at various facilities including hospitals, nursing homes, and clinics throughout the Manhattan territory. The Hospice/Palliative Home Care Planner is responsible for evaluating and establishing a plan of care for patients referred for home health services. As a key member of the business development and marketing team, the Hospice/Palliative Home Care Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners at the assigned locations. “Must-Have” Skills for the job: 1. Licensed with current Registration to practice as Registered 2. Professional Nurse in NYS is required 3. Bachelor's Degree preferred 4. Two years nursing experience in a hospital setting, preferably in the emergency room, critical care, oncology or telemetry with proficiency in physical assessment and interviewing skills 5. Business/Marketing background strongly preferred 6. Bilingual Spanish/English is preferred
