Registered Nurse - Home Care/Hospice Brooklyn, NY 12 hr/Shifts Days Job description: As the Homecare/Hospice Nurse you will provide end-of-life care to all patients accepted to our Hospice Program and admitted to our Hospice Residence in Brooklyn. You will assess, plan intervenes and evaluate all care provided to our patients. You will also provide patient and family education and document all care provided.
