RN Coordinator of Care (Queens)

Overview

RN- Coordinator of Care Queens, NY Shift: 12 hrs or 10 hrs Day Shift, rotating weekends. Job description: As the Coordinator of Care RN, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. This is essentially a Field RN position. You will go out into the patient's home to perform assessments, engagement, etc. Required skills & experience (the “must haves” to be considered): NYS RN License 1 year of any type of nursing experience EXCEPT peds/nicu/maternity CHHA experience preferred BSN Preferred
