Job Title: Home Health Nurse Job Description: Client is seeking a home health nurse to provide, coordinate, and direct the provision of home nursing care according to physician's orders based on agency policies and procedures, through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Required skills & experience 1. Current and unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work by the Company. 2. Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license. See list of Nurse Licensure Compact States 3. Minimum of 2-3 years of nursing experience required. 4. Current CPR certification. 5. Minimum 6 months of Home Health experience required What you need to know: *** Candidate's birth day, birth month, first 2 letters of last name and last 3 digits of social are required for submittal*** Position Title: REGISTERED NURSE I Total number of positions needed: 1 1) How long is the contract assignment? : 13 weeks 2) Schedule of assignment hours? M – F 8am – 5pm. Rotating one weekend a month, one week on call per month. (there are minimal calls) 3) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Yes can move to hire 4) What region are you in? Home health 5) What’s creating the request? - work load 6) To which location will this person report for assignment? Stamford, CT 203-327-2680 7) How many miles per week will the consultant travel? 20/40 a day 8) What are the weekly units of productivity expectations? 30 9) What EMR systems will the consultant be using: Homecare Home Base preferred, Oasis, Outlook 10) Will there be any on call? If so, how much? One week a month on call (minimal calls) 11) Do you want the candidate to have home health experience? Yes, minimum of 6 months
