Nurse - Care Manager

Overview

The Nurse Care Manager (NCM) will use nursing assessment and evaluation skills to guide treatment and care decision for the entire time. You will develop and implement a plan of care that addresses medical, psychosocial and other needs of your patient. You will also draw on critical thinking to help your patient navigate the health system, identify and overcome barriers to care, and draw from resources, services and vendors in the community. The position is predominantly office-based, although you may do home visits on occasion. Required Skills & Experience • Current RN License required, BSN preferred • 3-4 years of clinical practice in a hospital, clinic, home care, hospice or nursing home setting • Case management or care coordination experience required • Preference given to candidates with geriatric experience What you need to know - Everything is done from the office (no remote work) - Schedule is Mon-Fri 8-5 - Patient load - approximately 125 patients - Interview Process -Initial phone screen -Personality assessment to be taken before they interview with the hiring manager -In person interview with the hiring managers -Start Date: ASAP pending background check
