Job description: Bronx, NY LPN-Home Care Full time Day We are looking for a Home Care LPN to join our team. This LPN will be reporting to the Coordinator of Care and will be responsible for seeing patients in their home for revisits based on a skilled need. They will also implement the plan of care, evaluate the treatment and document the visit electronically (completing documentation within 24-48 hours). Qualifications: -NY State LPN License required -Minimum of one year LPN Med/Surg experience in a hospital or Longterm Care facility required -Must have a NY State Drivers License and a car -Good computer skills required
