MDS Coordinator

Overview

We would like a skilled Minimum Data Set (MDS) Coordinator to join our Skilled Nursing Care team. This person will be responsible for coordinating the completion of the resident assessment process including scheduling, completing, processing, transmitting and filing the MDS and associated reports. Coordinator is also responsible for scheduling and coordinating care plan conferences and care plan updates, closing medical record discharge charts in a timely manner. Qualifications/Requirements: 1. Current Indiana RN license 2. 2-3 years of MDS experience required 3. Must be experienced with insurance verifications, pre-authorizations and medical record up keeping 4. RAC certification is strongly preferred (Resident Assessment Coordinator)
