Nurse - Coach

Overview

Our facility is seeking a knowledgeable nursing coach in a telephone triage role. In this role, the RN coach will establish authentic personal relationships with patients and their support systems. The nurse coach will recommend primary and specialty care providers and community resources to enhance patient experiences while adhering to the facility’s commitment to patient-centered care. This role is part of the Care Connections team, and will collaborate with physicians, mid-level providers, and other staff members. Responsibilities in this role include helping patients navigate the healthcare system by answering general health questions, assessing healthcare access for the patient (using appropriate protocols), and promoting engagement to reduce risks of hospitalization. The nurse coach also will assist, develop, and coordinate self-management support including chronic care management and acute triage. Requirements: • Associates degree required, BSN preferred • 5+ years of critical care nursing experience required • North Carolina RN license required • Educator RN background preferred
