Overview
We are looking for an experienced RN to take the next step developmentally in their nursing profession! The team is looking for an RN MDS specialist with a strong leadership/management background to work full time! This position would be ideal for an RN that has worked in the field and looking at moving up and challenge themselves professionally to lead and be responsible for a whole team. This is a 192 bed skilled nursing facility providing care in a non-traditional way through 16 twelve bed houses. The MDS Registered Professional Nurse assumes the responsibility and accountability for the discharge planning process for all the patients assigned to her caseload. The MDS Nurse serves as a resource and expert to the staff in designated care centers and provides ongoing education to staff, patients, and families. Nurses in this position work within the interdisciplinary team concept to ensure the services needed for each patient to have a successful outcome for their stay here are provided in a timely and professional manner, as well as provided in accordance with State and Federal regulations. Enters MDS data and manages quality measures and risk issues. Monitors care plans and works with Educators. Minimum Requirements: • Valid NYS RN license • Associate’s degree in nursing • Current CPR and BLS • 2 + yrs of experience working in a skilled nursing facility as a RN • 1+ yr of experience in a leadership role overseeing staff • 1+ yr of MDS experience Preferred Requirements: -Bachelor’s degree in nursing -Highest level of customer service while completing day to day duties and responsibilities -Excellent communication in a fast pace work environment across teams on different shifts -Strong engagement within a team setting -Leadership and mentorship skills overseeing RNs, LPNs and C.N.A staff -Passion to learn, grow and develop in the nursing field
