Nurse - Home Care

Overview

Our Nurse and Rehab Association has a wonderful opportunity for a RN interested in home care and wanting to be mobile throughout the community in different counties throughout the capital district! We have openings for an RN to work full time in Albany or Rensselaer county. This position is ideal for nurses that are looking at starting and ending their day at home and care for one patient at a time! Full time schedule will be Monday - Friday 8-4p (minimal holiday rotation) The primary responsibilities of the role include new patient assessments and admissions, resumption of care visits for patients returning home from the hospital and revisits. Minimum Requirements: -NYS RN license -Associates degree in Nursing -2+ yrs of recent experience as a RN in acute care -Valid driver license and reliable vehicle Preferred Requirements: -1+ yrs of home health experience - Highly desired but willing to train the right candidate -Bachelor's degree in nursing - Maintain the highest level of customer service while completing day to day duties and responsibilities
