Community Health RN

Overview

Community Health RN M-F 8am-4:30 pm Syracuse, NY The Community Health Nurse is primarily an office position that is responsible for coordination of care, documentation of care, and some direct nursing care for agency clients. The Community Health Nurse communicates with internal and external referral sources (client, family, community agencies) to ensure all required information is obtained and confirmed for referral processing. The Community Health Nurse provides clinical support to the clinical field staff, Home Health Aides and Clinical Operations Manager. Requirements: -Current NYS Registered Professional Nurse license -Baccalaureate Degree in Nursing (or currently working toward BSN) from a regionally accredited or an approved college or university acceptable to NYSDOH -Minimum of 1 year nursing experience and 1 year experience in a home care setting -Current and valid driver’s license, auto insurance and a vehicle which can be used for work (eligible for agency vehicle 6 months after hire)
