Overview
The Director of Clinical Care Management is a key leader and will be responsible for ensuring the quality of our clinical care management services. The Director reports to the Chief Medical Officer and oversees the care provided by our clinical staff in the field from our central office. They should have a passion for excellent patient care and improving patients’ lives and be able to work in a high-ambiguity and self-directed environment. This leader will drive the process of developing, implementing and continually improving care management policies, while ensuring our clinicians are trained in those policies and practicing them effectively. The Director will be a subject matter expert on care management and will be current on the latest evidence-based guidelines, which will help them serve as both an internal resource as well as a clinical liaison to our many partners. Top "Must Haves" from the Account Executives: 1) Bachelor’s degree from a four-year college highly preferred 2) Current Tennessee (or other NLC participating state) RN License 3) 2-5 years previous experience working with patients with late stage Chronic Kidney Disease required 4) 2-5 years previous experience coaching, leading and hiring a top performing team is preferred.
