Job Title: Home Care RN - Private Pay Job Description: As RN Home Care Nurse you will provide direct care to clients and their families according to the established plan of care. As a RN Home Care Nurse you will provide a multidisciplinary approach to care. You'll also play a key role in developing a professional care plan from the assessment findings and in collaboration with the clients Physician to ensure the facilitation of the provision of the best possible care. Required skills & experience (the "must haves" to be considered): 1. Licensed Registered Nurse (RN) in the (appropriate state) with no disciplinary action 2. 1 year of Home Care experience , private pay home care experience through an agency required 3. Must be able to participate in our on-call rotation 4. Care Management Certification preferred 5. Candidate must live within Manhattan, NY What you need to know: 1. Incredible employee benefits 2.
