Overview

The Clinical Team Manager is responsible for organizing and directing the daily operations of a specific branch or office. This role will work with a dynamic, positive team that is passionate about delivering the very best in transitional care, will be a leader with excellent communication and problem-solving skills. The Clinical Team Manager will be responsible for the day-to-day management and leadership of their assigned field clinical operations team. The Clinical Team Manager will oversee Care Coordinators, High Risk Case Managers, and Social Workers, and will monitor their caseloads and recommend adjustments as needed. This role will also be responsible for hiring and onboarding new colleagues, and as well as conducting annual reviews and evaluations. In some cases, the Clinical Team Manager may act as a mentor or preceptor for new hires until the hire is able to work independently. Additionally, the Clinical Team Manager also ensures staff understand and adhere to established clinical protocols and processes and will conduct one-on-one meetings and observations throughout the year. This position may work with internal and external partners, local leadership and centralized operational teams to make sure processes remain in compliance and that members and providers have a positive experience. 
Requirements: • Registered Nurse, Registered Therapist (OT or PT), or Speech Language Pathologist required • Active Pennsylvania license required • 5 years of clinical management experience required • Certification in case management (Commission for Case Management Certification CCMC) required • Experience managing multidisciplinary care teams, including nurses, therapists, social workers, case managers required • Experience working with the geriatric population preferred • Knowledgeable of InterQual and Milliman preferred • Knowledgeable of NCQA and URAC standards preferred
