Job Title RN Hospice - Intake - Capital District Job Description Seeking an RN to work as a part of our Hospice Intake RN Team covering hospitals in the Capital Region (Samaritan, Albany Memorial, Ellis, St. Peter’s, AMCH). The RN Hospice - Intake will evaluate patients for eligibility and interest in hospice services, describes the support and care available from along with the benefits of hospice care. Coordinates with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services. Completes the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team including social workers, chaplains, volunteers, complementary therapists, home health aides and more! Ensures a smooth handoff to the hospice RN Care Manager. Required Skill and Experience - Associates degree in nursing, Bachelors preferred - 2 years + experience as a RN in an acute care environment - Valid driver license and reliable vehicle - Valid NYS RN license - Certification in Hospice and Palliative Nursing, or a commitment to successfully complete the certification exam within two years of hire - 1+ year of experience with case management, marketing and case finding, public relations
