Job Title RN Hospice - Admissions Job Description Seeking a full-time RN with recent acute care/hospice or home health experience to join our Hospice Nursing Home Admission Team in the Capital Region. We have a great team of RNs who work with our RN Supervisor, the patient's attending physician in the community, and our hospice Medical Directors to meet with patients and families in the nursing home to discuss hospice services, admit patients to our care, establish a care plan, arrange initial support, supplies and equipment, and transition information to the hospice RN Care Manager. Required Skills and Experience - NYS RN Licensure - 2+ years experience as a RN in an acute care environment - Reliable vehicle and driver license - 1+ year of experience with case management, marketing and case finding, public relations - Bachelor's degree in nursing - Strong computer skills utilizing multiple computer applications
