Job Title RN - Recovery Care Coordinator (Per Diem) Job Description Seeking an individual who is interested in being part of a fast-paced start-up culture at an organization leading the country in redefining the way we care for patients that typically would be treated in the hospital setting. As a Recovery Care Coordinator Nurse (RCCN), you will be at the forefront of this start up's highly innovative care model that allows patients the option to receive acute level care in the comfort of their own home in lieu of an inpatient setting. As a Recovery Care Coordinator Nurse, you will leverage the proprietary workflow platform, advanced telehealth technology, and our interdisciplinary network of doctors, nurses, social workers, and ancillary providers to improve the health and well-being of our patients during what is often one of the most challenging times in the lives of our patients and their families. Required skills & experience (the “must haves” to be considered) - ASN – Associate of Science in Nursing and current, active WI RN license in good standing - 3-5 years of acute nursing experience. Possess a general knowledge of clinical standards and outcome measurement. - Enjoys working within teams; dependable in both productivity and attendance. Flexible and willing to share on-call after hours with other Recovery Care Coordinators is required - Acute care experience - Certification: Care Coordination and Transition Management (C.C.C.T.M), preferred What you need to know - Company is in their late start-up phase.
