Overview

Job Description The Patient Navigator serves as the communication hub for the patient, caregiver and the clinical team providing care across the continuum for the patient. The Patient Navigator engages the patient prior to surgery and remains the patient’s point of contact for the duration of the episode of care. An effective Patient Navigator possesses excellent written and oral communication skills and the ability to work equally well with patients, caregivers, physicians and other health care providers involved in the patient’s care. This position requires the ability to build strong working relationships, solve problems, assist patients in communicating with their physician to obtain appropriate care resources and documents interactions appropriately. The Patient Navigator ensures all parties involved receive accurate/timely responses to questions/concerns. The position requires strong care management experience, including discharge planning experience. Required skills & experience (the “must haves” to be considered): - Educational Requirement: RN - Minimum three years in healthcare environment, case management and discharge planning experience as a Registered Nurse - Registered Nurse (RN), current unrestricted license is required - Orthopedic and/or cardiac experience working with adult populations - Bilingual: Spanish/English What you need to know: - Excellent base pay and benefits - Opportunity to work for a dynamic, quickly growing organization - Opportunity to speak with doctors and patients all over the country - Ability to spend time with patients and get to really know them
