Overview
Job Description The Care Coordinator II collaborates with the Clinical Case Manager to develop, implement and monitor the plan of care to determine if the goals are being met on an ongoing basis, to evaluate for needed changes and to update the plan of care accordingly. The role is to coordinate continuity of care for patients often as a liaison between the patient’s family and healthcare organization, ensuring that the proper treatment is administered at the appropriate time to maximize health and well-being while also minimizing the need for hospitalization. The Care Coordinator II strives to promote self-managed care and the use of healthcare resources in the most cost-effective way possible, working with patients of all ages and conditions, but primarily focusing on a specific population. This position integrates care coordination, utilization management, and discharge planning. Required skills & experience (the “must haves” to be considered) • Associate's, Bachelor’s, or Master's degree • At least 3-4 years of relevant professional experience, including various healthcare experience in case management, transitional care, and post-acute service. • LPN, LPC, BSW or MSW license required • Knowledge of disease pathophysiology, psychosocial issues, and treatment • Medicare and Medicaid experience required What you need to know - Awesome office location with onsite gym - Amazing opportunity with a fast-growing company based out of Oak Brook, IL - Great base salary with bonus potential, benefits and PTO allowance
