Physician - House Calls

Overview

Job Description The House Calls Program seeks a Primary Care, Family Medicine, or Internist trained physician to join our House Calls Team. This is a full-time position with our employed provider group. The House Calls Team provides primary and urgent care services, and hospital follow up to patients in their home or at assisted living facilities. In addition to House Calls, our providers can refer to Hospital at Home which was designed to provide hospital level care at home to reduce readmission to the hospital. Required skills & experience (the “must haves” to be considered): - BC/BE in Primary Care, Family Medicine or Internist What you need to know: - Expected to see 5-6 patients daily - Schedule: M-F 8:00am - 5:00pm, schedule may fluctuate - Rotate call per week, once every 4-5 weeks. After the 5th call, will receive phone coverage. - Relocation and sign-on bonuses available based on experience - Benefits start day one
