Hospice Care Planner Referral Manager (RN)

Overview

Required Skills and Experience (the 5 "must haves" to be considered) 1. New York State RN license 2. BSN degree required (Masters preferred) 3. At least 4 years of Hospice experience with prior managerial/leadership experience strongly preferred 4. CHPN certification preferred of must be achieved within the first year of hire 5. Prior marketing/business development experience preferred What you need to know: The Hospice Care Planner Referral Manager provides direction and supervision to a team of Hospice Care Planners covering an assigned area. The manager plans, coordinates, manages and evaluates the intake coordination process to ensure his/her team is providing accurate patient information to the Access Center in a timely manner to ensure that patients who meet hospice criteria are quickly enrolled onto the program.
