Required skills & experience (the 5 “must haves” to be considered) 1. Licensed with current Registration to practice as Registered Professional Nurse in NYS is required 2. Bachelor's Degree preferred 3. 2 years nursing experience in a hospital setting, preferably in the emergency room, critical care, oncology or telemetry with proficiency in physical assessment and interviewing skills 4. Hospice experience is a plus 5. Business/Marketing background strongly preferred What you need to know Working at various facilities including hospitals, nursing homes and clinics throughout Manhattan, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice services. The Planner will facilitate admission of referrals upon discharge from the hospital to a hospice home care or facility program that meets the needs of the patient and his or her family members. As a key member of the business development and marketing team, the Hospice Care Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners in the assigned area.
