Job Title Patient Care Coordinator Job Description The Patient Care Coordinator provides support to clinical field staff, the House Resource Manager, Clinical Supervisors and the Vice President of Patient Services through completion of scheduling, clerical tasks and coordination. Required Skills and Experience - High School Diploma or G.E.D., LPN preferred but not required, with a minimum 3-5 -year medical office/ customer service experience. - Computer proficiency with Microsoft office, or other word processing, spreadsheet, and database management. - Excellent organizational skills with the ability to prioritize multiple tasks. - Excellent customer service both to internal and external customers. - Ability to communicate effectively in English, both verbally and in writing. What You Need to Know - Provides administrative and technical support to House Resource Manager, Clinical Supervisors and other team members. - Responsible for maintaining availability of employees in scheduling system. - Responsible for supporting the Clinicians and Clinical Supervisors in the initial and ongoing scheduling of the patient visits for all disciplines including on-boarding clinicians. - Completes reconciliation of schedules for assigned team/clinicians/team HHA. - Answers telephone calls for team and directs to appropriate staff. - Track and disseminate information to appropriate department(s) regarding patients in facilities. - Participates in departmental educational in-service and team planning sessions. - Participates in appropriate agency meetings and quality improvement and utilization review activities of the organization.
