Overview
Required Skills and Experience ("must haves" to be considered) 1. LCSW or PsyD 2. Experience providing direct mental health treatment, substance use treatment, or both. 3. Experience (formally or informally) providing supervision to social workers, community health workers, or care coordinators, and are interested in growing your supervision skills. 4. Experience working with individuals with co-occurring chronic medical and chronic behavioral health diagnoses. 5. Direct familiarity with clinical settings. What You Need to Know - Follow a panel of members to provide short term therapy, clinical support, and crisis intervention. - Provide supervision for a cohort of Community Health Partners. - Provide care team support from a behavioral health perspective. - Coordinate services and relevant Member Action Plan (MAP) goals and tasks with the organization's care teams, as well as community service providers. - Partner with members in their homes, communities, and through the complicated paths of the healthcare system. - Work daily within our custom-built care facilitation platform, Commons, which will enable you to collect data, organize information, track tasks, and communicate with staff, patients, and family. This platform is built for a mobile workforce, and you will field-test our technology, provide feedback to the product development team, and, over time, become part of a super-user group to assist in onboarding and supporting others.
