Job Title Community Health Partner Lead Job Description In this role, you will join an inaugural cohort of staff providing direct services to our members through our innovative care model. You will be associated with one of our inaugural clinical “hubs,” a primary care practice that in many respects will look more like a community center than a doctor’s office. Your work will frequently be field-based, extending out in the surrounding neighborhood and community. It is integral to our care model that we meet people where they are, both physically and emotionally. As a Community Health Partner Lead, you will provide coaching, support and supervision to our Community Health Partners (CHPs), a team that serves as the heartbeat of our care model. In managing and leading a team of CHPs, you will support and develop them as they outreach, engage and guide members through their Member Action Plans (MAPs). You will collaborate with our Hub-based clinical leadership and Behavioral Health Specialists to support and build performance metrics for our CHPs, while facilitating daily supervision to your direct reports. Required Skills and Experience - You have experience providing supervision to social workers, community health workers, or care coordinators - You have direct experience supporting at-risk populations with complex needs - You are an independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model - You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care - You have experience training in motivational interviewing and conflict resolution - You you're familiar with community resources and social benefits (Nice to have) - Behavioral health licensure - including LCSW, LMSW, LMHC or PsyD - Direct familiarity with clinical settings - Training in problem-solving therapy - Multi-lingual and multi-cultural - Familiarity with Brooklyn What You Need to Know - Supervise a team of Community Health Partners, providing guidance, coaching, and training around engagement, care planning, and longitudinal care - Care for a small panel of members; act as a point person for these members and partner with them longitudinally in attaining their goals - Support your team in member outreach and engagement, helping them build comprehensive assessments of members’ multifaceted health needs (physical, behavioral and social) - Guide your team through development of member-driven, member-centered action plans (MAPs) that address social care needs - Model how to foster lasting and trusting relationships by doing so with your team and your members - Act as a point person for the members on your team’s panels: co-visit members with CHPs and provide assistance in crises Support your team to work with members longitudinally to achieve their goals and identify emerging needs
