Overview

Job Title Hub Registered Nurse Job Description In this role, you will be a part of the first cohort of staff providing direct services to members, who face economic and social challenges in addition to having mental and physical health needs, through our innovative care model. You will be working within our first clinical “hub,” a primary care practice that in many respects will look more like a community center than a doctor’s office. As an RN in our Hub, you will work with a panel of members to improve their health holistically. You will work alongside, and will be supported by, a care team associated with each member, whose work is focused around a Member Action Plan (MAP). You will engage members in the hub, facilitating their on-site care and being a bridge between on-site care and field-based care. Required Skills and Experience - You have a current, unrestricted NYS RN license. - You have experience providing clinical services to individuals with co-occurring chronic medical and behavioral health conditions. - You are an independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model. - You are excited about how technology can support your work and help drive the ongoing evaluation toward new and better care. (Nice to have) - Experience working collaboratively with a interdisciplinary care team, and specifically working alongside community health workers or care coordination staff. - Experience in home care, hospice/palliative care, acute care, and/or care management - Multilingual What You Need to Know - Act as the practice-based nurse, supporting on-site providers in facilitating an efficient practice that promotes member engagement, trust, and support. - Provide care team support from a nursing perspective, and act as a bridge between on-site care and field-based care. - Coordinate services and relevant Member Action Plan (MAP) goals and tasks both internally and with external providers. - Foster lasting and trusting relationships to assist members in achieving goals, identifying new needs, and coordinating care. - Meet with members in the hub as extenders of the primary care providers and other care team members., including medication reconciliation, standing orders, and disease management education and oversight. - Providing in person triage for members both in person and via phone.
