Overview
Required Skills and Experience 1. Bachelor's Degree in Nursing, Allied Health or related field. Master's degree preferred 2. Registered Nurse - OH license 3. Minimum of two year’s recent and relevant clinical and management experience required - Management in Hospice/Palliative care 4. Experienced in computer systems and Microsoft Office Functions, database management, office automation processes, and project management. What You Need to Know - Plans, develops, deploys and monitors outcomes related to the palliative care services continuum provided by an interdisciplinary team of clinicians. Establishes performance improvement targets and processes, providing oversight and monitoring for effective outcomes - Manages the performance of an interdisciplinary care team, whose members are assigned to the Palliative Care programs (social workers, nurses, nurse practitioners and physicians). Optimizes the allocation of the team resources to meet patient care demand within each program or location; manages schedule to assure coverage for patient care needs - Along with the Executive Director and Medical Director, serves as the subject matter expert, collaborating with hospital executives and leaders, health system executives and others to assure effective operation and impact of the palliative care programs - Orients new staff members in the department
