Top "must haves" to be considered -You have an LCSW -You have experience with a seriously mentally ill population, and other significant behavioral health diagnoses/concerns. -You enjoy crisis management and intervention. -You have familiarity with Care Management, primary care, and/or behavioral health programs and practices -You have the ability to work in a fast-paced, growth oriented environment where independence, flexibility and adaptability are required -You have the ability to manage multiple projects -You have the desire to contribute to a learning and evolving model of care, and a quickly growing staff -You are excited about how technology can support your work and help drive the ongoing evaluation toward new and better care What you need to know: -Work closely with the care team (especially Community Health Partners and Behavioral Health Specialists) on identifying HARP members who are interested in Home and Community Based Services. -Work with care team members to explain the HCBS process to members, and design a process that is most comfortable with the member. -Complete the HCBS eligibility assessment independently or with the existing care team depending on the member’s preference, and using the assessment to develop the HARP plan of care alongside the member and care team. -Collaborate with Beacon and HCBS providers for members connected to the services, until at least the member’s second visit with the HCBS provider. At this point, the member’s continued support will be provided by the Community Health Partner and the rest of the care team. -Be available ongoing to support community health partners and other care team members around HARP and HCBS. -Support the Behavioral Health team by assisting with behavioral health evaluations.
