Overview
Required Skills and Experience 1. You have a current, unrestricted North Carolina RN license. 2. You have experience providing clinical services to individuals with co-occurring chronic medical and behavioral health conditions. 3. You are an independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model. 4. You are excited about how technology can support your work and help drive the ongoing evaluation toward new and better care. 5. You have an unrestricted North Carolina driver's license and vehicle What You Need to Know -Act as the practice-based nurse, supporting on-site providers in facilitating an efficient practice that promotes member engagement, trust, and support. -Provide care team support from a nursing perspective, and act as a bridge between on-site care and field-based care. -Coordinate services and relevant Member Action Plan (MAP) goals and tasks both internally and with external providers. -Foster lasting and trusting relationships to assist members in achieving goals, identifying new needs, and coordinating care. -Meet with members in the hub as extenders of the primary care providers and other care team members., including medication reconciliation, standing orders, and disease management education and oversight. -Providing in person triage for members both in person and via phone.
