Overview

Required skills & experience (the 6 “must haves” to be considered): 1. Current professional nursing licensure in the state of IL required 2. 2+ years of clinical nursing experience required. Critical/Acute care nursing is preferred** 3. Minimum two (2) years of utilization review, discharge planning, case management or disease management strongly preferred. Home health nursing is NOT the background the client is looking for in a RN. 4. IDEAL CANDIDATE will be someone with acute/critical care and utilization review/management experience. 5. Must have emotional maturity to communicate, educate, engage and care for patients and their families. 6. Reliable means of transportation and valid IL drivers license required. Care Navigator will be spending time in the hospital and then out in the field visiting patient's in their homes. What you need to know: • This is a NEW role and the client needs to full the positions ASAP. • Case load: 50-55 (may change depending on territory) • Monday-Friday (8:30am-5:00pm) or (9:30-6pm), with every third weekend rotation required. • Call is one week at a time, every 4-6 weeks • Connect with patient at the time of diagnosis (either ED or pre-admission) and follow the patient across the hospital stay and 90 days post discharge • Guide the patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patient with navigating the continuum of care.
