Overview
Required skills & experience (the 5 “must haves” to be considered): 1. Masters in Social Work (MSW) required from a graduate school approved by the Council on Social Work Education 2. LMSW/LSW Required 3. At least 2 years of medical social work experience 4 Familiar with medical case management and healthcare 5. Strong working knowledge of Milliman Care Guidelines What you need to know: • Provide onsite and telephonic care management services to individuals identified as “high risk members” • Will travel between the office, member's homes and acute and subacute facilities • Will participate in twice weekly meetings with Medical Director to review member’s progress, update plan of care as needed • Work Monday through Friday- no nights, weekends or holidays • Full medical, dental, and vision benefits • Unlimited PTO and fully stocked kitchens
