Overview

Required skills & experience (the 5 “must haves” to be considered): 1. Registered Nurse with current, active unrestricted CA licensure required (**TCC Role will only accept candidates with a RN license**) 2. 3-5 years of RN experience with an acute/critical care, rehab/long term acute care or Post-Acute Care (PAC) background 3. Experience transitioning/discharging patients from acute (required) to Skilled Nursing Facility (strongly preferred) 4. CMS and managed care knowledge preferred. 5. Excellent documentation and technology skills required What you need to know: • The Transitional Care Coordinator must live within a 30-minute commute of Modesto, CA. • The Transitional Care Coordinator is responsible for identifying the appropriate Post-Acute Care (PAC) setting and evaluating a defined population for transitional needs post-discharge to improve outcomes. • Coordinators will manage an average caseload of 25-30 patients • 80% of the day spent in the field providing onsite support to patients across several hospitals and skilled nursing facilities • 20% of day spent in home office-documentation, e-mails, prep (start this at end of the day) • Will have six weeks of training; the first two weeks will be classroom training in TN (travel expenses will be covered)
