Required Skills and Experience: 1. Licensure requirements of the state of jurisdiction; North Carolina 2. Board Certified in Family Medicine, Internal Medicine, Geriatric Medicine 3. Primary Care experience required, home-based Primary Care is a plus 4. Fellowship training in Geriatrics, Addiction Medicine, and Internal Medicine preferred What you need to know: 1. Patient population: complex, chronic, geriatrics 2. New, innovative tech model that creates long-term lasting relationships with patients. 3. Seeking a candidate who is interested and excited about new technology and care models. 4. Our mission is to make healthcare more effective and affordable for all, including the underserved. Business model is twofold: virtual care side/telemedicine where patients can interact with their providers and field-based side where vulnerable, complex patients will receive home care.
