Overview
Required skills & experience (the 5 “must haves” to be considered) 1. Masters in Social Work (MSW) required from a graduate school approved by the Council on Social Work Education 2. LMSW/LSW Preferred 3. At least 2 years of medical social work experience 4. Required-discharge planning experience and familiar with medical case management and healthcare 5. Technologically savvy and excited about how technology can change how we deliver care What you need to know • In coordination with nursing develop an outcome-based plan of care, based on the member’s input, with anticipated targeted dates for completion. • Identify and addresses biopsychosocial needs of the members and family • Work Monday through Friday- no nights, weekends or holidays • Full medical, dental, and vision benefits • Unlimited PTO and fully stocked kitchens
